Unemployment Power of Attorney

To designate ADP as your unemployment claims processor, complete and return the required POA authorization for each state in which your company pays wages and return to:

Alabama accepts ADP Form 6018. A state-sanctioned notary public must notarize signature(s) on this form. Form must be an original. Copies or faxes are not accepted.


Arkansas accepts ADP Form 6018. A state-sanctioned notary public must notarize signature(s) on this form. Form must be an original. Copies or faxes are not accepted.


California if a current Form DE 1 is on file with the state, the signer of the POA must be a Corporate Officer listed in section G of the current Form DE 1. A copy of Form DE 1 on file with the state must accompany the POA. If the client has not completed a Form DE 1, the client must complete one and submit with the POA Form 6018. The person signing the POA must be listed as a corporate officer in section G of the Form DE 1.


Colorado state Form UITL-18 is required. All fields must be complete and all signatures must be dated less than six months prior to submission. A state-sanctioned notary public must notarize signature(s) on this form. Form must be an original. Copies or faxes are not accepted.


Delawareletter of addressee stating that ADP is the address of record must accompany ADP Form 6018. A state-sanctioned notary public must notarize signature(s) on this form or the form must have an embossed corporate seal. Form must be an original. Copies or faxes are not accepted.


Florida state Form DR 835 is required. Faxes are accepted. Section 1 and Section 8 must be completed by the employer.


Hawaii accepts ADP Form 6018. This form must have an original signature of a corporate officer, therefore the form must be an original. Copies or faxes are not accepted.


Idaho accepts ADP Form 6018. Employers that wish to grant ADP access to view benefit charge information online must visit the agency Web site at http://labor.idaho.gov/dnn/Default.aspx?alias=labor.idaho.gov/dnn/idl and assign ADP as their agent.


Indiana employers that wish ADP to be addressee of record for their unemployment compensation (UC) documents must visit the agency web site at www.in.gov/dwd and assign ADP as their agent to handle their UC matters. Please click here for step by step instructions.


Minnesota employers that wish ADP to be addressee of record for their unemployment compensation (UC) documents must visit the agency web site at www.uimn.org and assign ADP the agent role to handle their UC matters. Please click here for step by step instructions.


Nevada accepts ADP Form 6018. This form must have an authorized signature by a current officer of the corporation (President, Vice President, Treasurer or Secretary), partner, managing member, owner, Chief Executive Officer, Chief Financial Officer, or the fiduciary of a trust or state.


New Hampshire accepts ADP Form 6018. Form must have an original signature of a corporate officer. Form must be an original. Copies or faxes are not accepted.


New Jersey state POA is required. A state-sanctioned notary public must notarize signature(s) on this form and the form must have an embossed corporate seal. (If a corporate seal is not available, indicate that fact on the POA prior to it being notarized.) Form must be an original. Copies or faxes are not accepted. NOTE: The state POA does not have a specific form number.


North Carolina state Form NCUI 604 must accompany ADP Form 6018. This is only required if the client does not already have a Form NCUI 604 on file with the agency with a matching signature. (We suggest that a completed Form NCUI 604 be submitted with each ADP Form 6018.) A state-sanctioned notary public must notarize signature(s) on ADP Form 6018. Form must be an original. Copies or faxes are not accepted.


Ohio state Form JFS-00501 is required. This form must have an original signature of a corporate officer, therefore, the form must be an original. Copies or faxes are not accepted.


Oklahoma state Form OES-190B is required. Signature(s) on this form must be notarized by a state-sanctioned notary public. Form must be an original. Copies or faxes are not accepted.


Pennsylvania state Form UC884 is required. An authorized employer representative signature is required. The POA must be submitted within three months of the signature date. Form must be an original. Copies or faxes are not accepted.


South Dakota state POA must be used. Signature(s) on this form must be notarized by a state-sanctioned notary public or the form must have an embossed corporate seal. Form must be an original. Copies or faxes are not accepted.


Texas state Form C-42 is required. A state-sanctioned notary public must notarize signature(s) on this form. Form must be an original. Copies or faxes are not accepted.


West Virginia accepts ADP Form 6018. A state-sanctioned notary public must notarize signature(s) on this form and the form must have an embossed corporate seal. If a corporate seal is not available, indicate that fact on the POA prior to it being notarized. Form must be an original. Copies or faxes are not accepted.


Wisconsin accepts ADP Form 6018. A state-sanctioned notary public must notarize signature(s) on this form or the form must have an embossed corporate seal. Form must be an original. Copies or faxes are not accepted.


All Other States that are not referenced individually will accept ADP Form 6018 with a signature and printed name of the corporate officer or owner.
